NL Athletics Participation Forms

Every year Athletes must complete all the UIL forms in order to participate in sports. In the past 4 years this information was filled out on our Dragonfly max software. As of May 2022, all online forms will be located on RegisterMyAthlete.com. Please follow the below instructions on how to set up an account. If you have any problems, watch the below video (Video will go on website May 20, 22) or talk to the Athletic Trainer at North Lamar.

Please know this is a new process for us, we hope this platform is easy with no bugs, but its a learning experience for us all. If you find problems or get stuck, reach out to the Athletic Trainer in person or cbarker@northlamar.net for assistance.

Athletic participation requires many rules and many forms. Below is a list of common forms used for all of our sports.

Physical Examination Form (Must be completed EVERY YEAR)

The Texas UIL Physical Must be on file and approved by the Athletic Trainer before any athletic participation at tryouts, practice, or game can occur for any UIL sport. This physical is designed to keep your athletes healthy and reduce the risk of injuries in sports by making sure they are capable of playing at an intense level. Please review the below facts:

This pyhsical must be completed every year 7^th grade and up. Local School Policy
There is a free physical hosted every year in May(typically at Paris ISD High School A Gym)
Only forms filled out in its entirety will be approved with student, parent, and pyhsician siganture and date.
Do not wait till the day of to get a physical. Try to get the physical a week or more in advance.
Physicals have to be turned in as a hard copy in good condtion. If the paper is torn or riped we will not accept it.
